Reducing social isolation in residential age care

Currently when someone is entered into residential age care only their medical/ health needs are assessed. A royal commission mandated the need for a social assessment at intake. 

This is about responding to front-line worker's requests for a tool or toolkit that assists in a more person-centred assessment ‘conversation’ that will offer a greater degree of understanding of the residents and their social connections. 

Social Connection Assessment Tool for Residential Aged Care Facilities An evidence based, co-designed assessment tool that draws upon previous research conducted by the Social Innovation Research Institute at Swinburne University of Technology.

3.Implement pilot in 2 sites
